Memory loss changes daily life for the whole family. Care is delivered by Personal Support Workers (PSWs) certified in Gentle Persuasive Approaches (GPA®) and U-First!® dementia care, and overseen by a Nurse Manager. Every plan is built around the individual: the stage they’re in, what they still know, love, and trust, and the abilities they can still draw on. Working in close collaboration with the Alzheimer Society of York Region, we keep our approach aligned with current, person-centred best practices. The result is fewer hard moments and a home that feels safe.
What’s Included
-
Personalized to Stage, Interests & Abilities
Care is matched to where your loved one is today: their stage of dementia, the interests and routines that still bring them joy, and the tasks they can still do for themselves. As things change, the plan changes with them.
-
Familiar Routines
We learn your loved one’s history and habits, then build the day around what feels normal to them.
-
Safety Supervision
Wandering risk, kitchen safety, and fall prevention, quietly managed without making the home feel restrictive.
-
Gentle Redirection
Certified techniques for confusion, agitation, and sundowning that protect dignity in difficult moments.
-
Cognitive Engagement
Music, photo albums, games, community outings, and conversation prompts chosen around what your loved one enjoys, to keep the mind active.
-
Personal Care with Patience
Bathing, dressing, and toileting handled at the client’s pace, not a schedule.
-
Family Education & Coaching
We help the family understand the stages of dementia and what to expect next, and give you the practical skills to manage each one.
A guide for families, stage by stage
You don’t have to figure this out alone. Dementia progresses in stages, and each one asks something different of families. Part of our job is to prepare you for what’s ahead and coach you through the skills that make each stage more manageable, so you feel confident, not caught off guard.
Early stage: preparing and planning. We help you establish routines, set up the home for safety, and put plans in place while your loved one can still take part in decisions. You’ll learn how to support independence without taking over.
Middle stage: managing daily change. As communication and daily tasks get harder, we coach you on the practical skills that matter most: calm redirection, responding to repetition and confusion, easing agitation and sundowning, and adapting activities to what your loved one can still enjoy.
Later stage: comfort and dignity. We guide you through more hands-on personal care, comfort-focused approaches, and connecting with the right support so no one in the family carries it alone.
Alongside our own coaching, we’ll connect you with community resources, including the Alzheimer Society of York Region, with whom we work closely, so your family has support at every step.
Care that stays current
Dementia care keeps advancing, and so do we. Our team’s certifications, Gentle Persuasive Approaches (GPA®) and U-First!®, are kept up to date, our care follows recognized person-centred, best-practice standards, and we invest in ongoing education so your family always benefits from current thinking. We work in close collaboration with the Alzheimer Society of York Region, where our CEO Michelle serves as a Board Director and chairs the Quality, Safety and Performance (QSP) Committee, keeping us connected to the latest guidance, quality standards, and community supports.
